Centra has received national and state recognition for quality with awards from HomeCare Elite and the Virginia Health Quality Center.
Centra Home Health has been named for the second year in a row to the HomeCare Elite, the only performance recognition of its kind in the homecare industry. 2007 HomeCare Elite is a compilation of the most successful Medicare-certified home health care providers in the United States. Centra Home Health's performance measurements in quality outcomes, quality improvement and financial performance place Centra Home Health among the top 25 percent of more than 9,200 home health agencies in the United States.
Centra Home Health also has earned the Home Health Outstanding Achievement Award from the Virginia Health Quality Center, honoring its excellence in meeting specific achievements in medical care. This award recognizes Centra Home Health for excellence in reducing avoidable hospitalizations, improving patient management of oral medications, target setting and immunization assessment.
“We credit the dedication, careful attention to each patient and strong quality program of the Centra Home Health team with this achievement and top quality performance,” said David Adams, Centra senior vice president. “We are especially proud of having the lowest re-hospitalization rate in this area. These awards recognize Centra Home Health professionals and the difference they make every day in the lives of thousands of home health patients in our region.”
Centra Home Health was one of only eight Virginia agencies to be awarded the Home Health Quality Achievement Award, given for accomplishment of a required level of quality measures from September 2006 to August 2007.This award was the result of a quality study with the Centers for Medicare and Medicaid (CMS) Home Health Quality Initiative with 149 Virginia home care agencies participating.
The 2007 HomeCare Elite Award is compiled by Outcome Concept Systems, Inc., the US leading provider of healthcare informatics, and DecisionHealth, publisher of home care’s newsletter, Home Health Line.
