Orientation plays an important role in ensuring you receive and understand the information you need to succeed as a student at Centra, and to provide safe quality care to our customers.
After completion of this form, click on submit below. Send your paper copy of the Confidentiality Agreement AND, IF REQUIRED your Non-Employee Access Request Form to your sponsor for validation.
Students: It is your responsibility to follow up with your sponsor and verify they receive an email confirming your General Orientation is completed.
