Community Health Needs Assessment & Implementation Plans

To provide a deeper understanding of our communities’ health needs and how we can address them, every three years Centra conducts Community Health Needs Assessments (CHNA). The completion of our Community Health Needs Assessments and successful implementation of our resulting action plans ensure that we are compliant with requirements for tax-exempt hospitals as created by the Patient Protection and Affordable Care Act and as promulgated in regulation by the Internal Revenue Service (IRS). The 2021 findings have been completed and we are happy to share these reports with you!
