Why We Post the CHNA

The Centra Health organization understands the importance of having a healthy community and we strive to connect to community members in order to understand their needs, what is important to them, and to develop ways in which we can help.  Our Community Health Needs Assessment (CHNA) is a continuous process for evaluating the health needs of the communities served.  The reasons for doing this are varied, but the most important reason is to help us realize our overall mission and vision to improve the health status of the community.  We use the CHNA as a key driver to our strategic planning process to help us design and implement new programs and services in response to the identified unmet community health needs.  The completion of our community health needs assessment and successful implementation of our resulting action plan will allow us to be in compliance with the CHNA requirements for tax-exempt hospitals as created by the Patient Protection and Affordable Care Act and as promulgated in regulation by the Internal Revenue Service (IRS).

We publish our CHNA and Implementation Strategy in order to inform the community as to the actions taken by the Centra Health organization in order to achieve our goal of a healthy community.  We encourage you to reach out to us and let us know your concerns, comments, and suggestions for ways Centra Health can continue to improve the health and well-being of our communities.  Your voice is important and we are listening!
