These free online assessments provide step-by-step short questionnaires that are easy to complete. At the end of the assessment, you will receive personalized, strictly confidential information that will help you:

  • Assess your current health status and identify those medical or lifestyle conditions that may lead to development of a disease
  • Take action to reduce your level of risk, including making you aware of the many services available through Centra

Prevention and Wellness Programs

N.E.W. Program - Eat More, Weigh Less

Healthy Steps is personalized structured exercise program aimed at preventing chronic disease and reduce health risks. This fitness program is developed by a registered nurse and/or exercise physiologist. Healthy Steps is designed to meet individual needs, including:

  • Obesity/weight management
  • Vascular disease (PAD/Stroke)
  • High blood pressure
  • Smoking
  • High cholesterol
  • Depression/anxiety/stress
  • Pre-diabetes, diabetes
  • Metabolic syndrome/insulin resistance

Healthy Steps

This class is a free one hour long program for people interested in considering or pursuing a tobacco free life.  It's not just tobacco anymore; juuling, e-cigs, and vaping are on the rise and this class can help you break those habits as well by providing education on cessation techniques that can be used to kick the habit for good!  Click here for more information!

Primary Care

The best way to prevent future disease or illness is through an annual physical with a primary care physician as well as regular check ups with your cardiologist. When you meet with your primary care doctor regularly, they can recognize any changes in or threats to your overall health!
