Guggenheimer Health and Rehabilitation Center is a130-bed health and rehabilitation facility located in the historic Diamond Hill district of Lynchburg.  Our focus is to provide you with the care you need to recover and return home, at your pace, after illness, hospitalization or  surgery.  We offer physician-directed skilled care nursing services and rehabilitation that will help you or your loved one regain strength, endurance, mobility and independence.  

The American Health Care Association reports that Guggenheimer patients are less likely to have to return to the hospital during their skilled care stay. This means that Guggenheimer has the care you need to get you home as quickly as possible. The dedicated medical team includes a full-time onsite medical director and nurse practitioner, RNs, LPNs, CNAs and a full team of rehabilitation and support staff all focused on your well-being and recovery.

Specialty Programs

  • Cardiac/Congestive Heart Failure - Guggenheimer has a specialized program providing education, nutritional support and rehabilitation services specific to the needs of patients with congestive heart failure. We coordinate our care with the services provided by Stroobants Cardiovascular Center at Centra Lynchburg General Hospital and provide weekly follow up with a cardiology nurse practitioner.
  • Diabetic Management - patients with diabetes benefit from a specialized program which focuses on helping individuals gain prior levels of diabetic management through education, nutrition and medication management.

Guggenheimer Health & Rehabilitation Center has been recognized for its dedication to continuous quality improvement with the American Health Care Association and the National Center for Assisted Living Quality Award (2011).
The award recognizes Guggenheimer for its organization-wide achievement in quality, performance outcomes, core values of visionary leadership and for their commitment to resident-focused excellence.  

Guggenheimer has received a 4 out of 5 overall rating from CMS of the Dept of Health and Human Services in its semi-annual ratings. This score means that our facility is rated above average overall based on health inspections, nursing home staffing and quality measures.  Guggenheimer is a member of the American Health Care Association and the Virginia Health Care Association.
