Community Health

Centra is a mission-driven, values-based, community–oriented healthcare organization committed to improving the health and well-being of the communities in which it serves.

Health starts in our homes, schools, workplaces, neighborhoods and communities. Taking care of ourselves by eating well and staying active, not smoking, getting recommended immunizations and screening tests, and seeing a doctor when we are sick all influence our health.  Health is also determined in part by access to social and economic opportunities such as quality schools, stable housing, accessible transportation and healthy foods.   These conditions are known as social determinants of health. 

To provide a deeper understanding of our communities’ primary health needs, and to develop implementation strategies to address these needs, every three years Centra conducts Community Health Needs Assessments (CHNAs).   We use CHNAs as key drivers to our strategic planning process to help us design and implement new programs and services in response to the identified unmet community health needs.

We also use CHNA to inform sponsorship decisions and award grants to nonprofit organizations who are helping to address the social determinants of health. Our outreach in the community also extends to supporting community health fairs and providing speakers on healthcare topics for community groups.
