Eligibility by Zip Code for PACE Lynchburg

To be eligible to participate in the PACE Lynchburg Program, individuals must be age 55 or older, qualify for nursing facility level of care in the state of Virginia, be able to live safely in the community with the support of PACE and live in a PACE Lynchburg service area as defined below.
