PACE

PACE is a Program of All-Inclusive Care for the Elderly. Living at home: your goal, our priority. PACE is designed to deliver all needed medical support for seniors ages 55+ that are eligible for nursing facility care.

Our approach to care

Services are delivered by a team of qualified, compassionate professionals made up of a primary care physician, registered nurses, a masters level social worker, health aides, transportation personnel, rehabilitation therapists, dietitians and recreation/activity personnel. The team works with participants and their families to develop a tailored plan of care which is monitored closely and updated as needed.


Our experts have an emphasis on the following:

  • Medical care and transportation to and from specialists
  • Adult daycare
  • Hospital care (as needed and coordinated by the PACE interdisciplinary care team)
  • Prescription medicines
  • 24-hour on-call physician services
  • Vision, hearing, and dental care
  • Rehabilitation and therapy services
  • Wheelchair-accessible transportation
  • Support from our interdisciplinary care team
  • Personal emergency response
  • Outpatient medical services
  • Mental health care
  • Home care and nutritional services
  • Respite services
  • Meals

Participants may be fully liable for the cost of services obtained without the authorization of the PACE Team (except for emergency services).

Centra's team of Caregivers is dedicated to providing the most advanced, personalized care.

Are you or your loved one eligible for PACE?

  • Age 55+
  • Qualify for nursing facility level of care in the state of Virginia
  • Able to live safely in the community with the support of PACE
  • Live in a PACE service area as defined on locations

PACE Pluses

Participants can be picked up from their homes each morning and brought to the PACE Center. They participate in social activities, eat a hot lunch with friends, and receive medical assistance as needed from a dedicated, compassionate care team. At the end of the day, the PACE van takes the participants back home to enjoy family, friends and the surroundings they know and love.
